LATHAM, N.Y. – March 3, 2026 – The CDPHP® Foundation is proud to announce the recipients of its 2026 grant cycle, recognizing three exceptional organizations committed to expanding access to vital health care and support services across our community.
The CDPHP Foundation, a public 501(c)(3) nonprofit affiliated with CDPHP, is funded through individual contributions and community‑supported fundraising events. This year’s grantees reflect the Foundation’s mission to promote health, wellness, and education through philanthropy and service. The 2026 funding focus prioritizes programs that improve access to physical and mental health care or remove barriers that prevent individuals from receiving the care they need. Each awarded organization has demonstrated a clear and measurable impact on expanding health care access for underserved populations.
2026 Grant Recipients
Astor Services
Astor Services will receive funding to help eliminate transportation barriers for clients seeking care at clinics and through the Partial Hospitalization Program. Across the region, limited transportation options have deepened inequities and delayed treatment. This investment will support reliable transit solutions for a portion of the more than 9,000 children, adults, and families Astor serves annually through its prevention, early childhood, special education, and behavioral health programs across the Hudson Valley and surrounding communities.
Captain Community Human Services
Captain Community Human Services has been awarded funding for its Wellness Express Medical Transportation Program, a critical resource for older adults in Saratoga County who lack reliable transportation. Many seniors aged 60 and above face mobility challenges, live on fixed incomes, and reside in areas with limited or no public transit. These barriers often lead to missed appointments and poorer health outcomes. Wellness Express provides free, door‑to‑door, non‑emergency transportation within a 50‑mile radius for primary care visits, specialty care, mental health services, diagnostic testing, and preventive or follow‑up appointments, helping seniors maintain better health and greater independence.
Veterans & Community Housing Coalition
The Veterans & Community Housing Coalition will use its funding to establish an offsite food pantry designed to increase access to fresh, healthy food for veterans. The expanded space will allow for greater shelving, refrigeration, and freezer capacity, providing consistent food availability. The pantry will also serve as a therapeutic volunteer opportunity for veterans living with mental illness, offering meaningful roles such as inventory management and distribution support. The organization currently supports veterans across seven counties in the Capital Region and beyond.
