MILFORD, Del. — Long before Delaware unveiled a sweeping strategy to strengthen rural healthcare, Milford was already demonstrating how integrated, community-based care can work.
The Milford Wellness Village, a locally rooted healthcare campus serving older adults and families, has been publicly highlighted by state health leaders and rural health advocates as an example of the kind of coordinated care model Delaware hopes to expand as part of its broader rural health strategy.
While the state’s recently released Rural Health Transformation Plan outlines system-wide goals rather than naming specific facilities, officials and advocates have pointed to the Milford Wellness Village as a real-world illustration of how bringing services together can improve access, reduce fragmentation and keep care closer to home.
For Milford residents, the recognition reinforces something many already believe — their community is quietly helping shape the future of rural healthcare in Delaware.

The Milford Wellness Village brings a range of healthcare and support services together on a single campus, including skilled nursing, rehabilitation, outpatient care and other wraparound services designed to support patients and families across different stages of life.
Lon Kieffer, executive director of the Milford Wellness Village, said the strength of the model lies in collaboration.
“When healthcare providers work side by side — sharing information and coordinating care — patients get better outcomes,” Kieffer said. “That’s true for seniors, but it’s also true for caregivers, working parents, people managing chronic conditions, and families who need help navigating the healthcare system.”
Kieffer said the integrated approach helps reduce barriers that often prevent people from seeking care.
“Instead of disconnected appointments in different locations, people receive care that’s coordinated, supportive and easier to access,” he said. “That leads to better health, fewer emergencies and a better experience for patients and families.”

Meir Gelley, who played a central role in developing the campus, was honored in 2025 with the Community Star Award during a National Rural Health Day event recognizing individuals who demonstrate exceptional commitment to improving healthcare access, innovation and community partnerships in rural areas.
According to organizers and public officials, Gelley was recognized for expanding services for older adults and underserved families and for helping establish the Milford Wellness Village as a community-based model of integrated care. Leaders also cited his efforts to address provider shortages, transportation barriers and healthcare gaps across Kent and Sussex counties.
A Local Success With Statewide Relevance
Delaware’s Rural Health Transformation Plan focuses on improving access to care in underserved areas, strengthening the healthcare workforce and reducing reliance on hospital emergency rooms — challenges that are particularly acute in rural communities.
Health officials have said models that coordinate services locally, similar to what has been developed in Milford, align closely with those goals.
Milford Mayor Todd Culotta said the Wellness Village reflects a broader vision for the city’s future.
“My goal is to make Milford a center for healthcare in Kent and Sussex counties,” Culotta said. “What’s happening at the Wellness Village shows the role Milford can play in meeting the region’s healthcare needs.”
